Abstract

The rapid transformation of healthcare systems through digital health technologies has paved the way for healthcare professionals to improve the quality of care and health outcomes among patients. Hypertension is a chronic healthcare issue impacting a large population of older age. There is a pressing need for chronic care to facilitate consistent management of hypertension. As primary care providers for long-term care involving patient care and education, nurses can play a vital role in improving healthcare delivery outcomes for hypertensive patients. The nursing informatics tool utilized for data analysis and trends is the use of Electronic Health Records (EHRs). This tool can help nurses analyze the trends and patterns among patients experiencing elevated blood pressure and practice follow-up care. As follow-up care is crucial for hypertensive patients, nurses can use this tool to find gaps in care and the need for appropriate interventions to bridge these gaps.

The literature has also shown the effectiveness of EHR in analyzing follow-up care trends among the hypertension population. Through this tool, healthcare professionals including nurses can tailor interventions that implement a change in the hypertensive population by promoting follow-up care and adherence to chronic care treatments. They can implement the ADKAR model of Awareness, Desire, Knowledge, Ability, and Reinforce to facilitate this change within the affected population and enhance follow-up treatment. This will include monitoring blood pressure, adjusting medication, and continuously educating patients on self-management behaviors and dietary management. Nurses can foster this desire for change by raising awareness through appropriate knowledge.

  • This poster aims to integrate an informatics model for hypertension. Hypertension (HTN) is a chronic healthcare issue where a patient encounters high Blood Pressure (BP) due to increased force of blood flowing through the blood vessels. Hypertension occurs when the blood pressure is 130-139/8-89 mmHg, known as type 1 hypertension (American Heart Association, 2019).
  • Hypertension impacts a large population, particularly old people, as the study describes that about 60 % of the population acquires HTN as they reach their sixties. Furthermore, about 75% of women and 65% of men have high BP in their seventies (Oliveros et al., 2019). Therefore, it important to scrutinize the data trends of hypertension and improve their health outcomes by utilizing healthcare informatic models.Nurses are the primary caretakers of chronic diseases like hypertension as they require long-term care and management. With time, healthcare information technologies are evolving rapidly, improving nursing practices in hypertension care. The American Nurses Association (ANA) promotes the use of health information technologies by nurses in improving hypertension care. The health outcomes of hypertension among older people can be effectively improved as nurses utilize informatic models and tools to promote coordinated care among affected patients (Koopman et al., 2020).

Tools for Data Analysis and Management

 Nurses can use informatics models or tools such as Electronic Health Records (EHRs) for hypertensive patients in old age. These informatics tools enable real-time monitoring, trend analysis, and predictive modeling for hypertension management (Koopman et al., 2020).  

By leveraging informatics tools, nurses gain actionable insights, allowing for timely interventions and personalized care plans. 

Literature Review

The study by Lu et al. (2020) utilizes the informatics tool of EHR to evaluate and assess the data trends on hypertension and patients who are undergoing a follow-up visit. The graphs below indicate the data trends for patients with elevated blood pressure and subsequent check-ups.  This helps nurses assess the gap existing in care treatments for hypertensive patients and highlights the needs and opportunities for follow-up support to achieve optimal health outcomes in patients with hypertension.

The study by  He et al. (2023),  guides nurses in using EHR as an informatics tool to assess hypertensive patients’ follow-up care patterns. As hypertension requires frequent follow-up for adjusting their medications based on the overtime monitoring of blood pressure, nurses must implement this tool to improve healthcare outcomes for patients and keep a follow-up appointment intervention for these patients. 

Model for Change

The ADKAR model is appropriate for implementing a change management strategy within an organization that addresses the healthcare issue of hypertension and the need for appropriate follow-ups to improve health outcomes (McGhee, 2023)

Nurses can use this model to cultivate the need for follow-up care for hypertensive patients in older age in the following ways:

They can raise awareness about hypertension, the need for chronic care for improved health outcomes, and the prevention of hypertension-related complications such as stroke, heart attacks, and death (American Heart Association, 2019).

Nurses can also create a desire for change in enhancing follow-up care for hypertensive patients by highlighting the positive aspects of follow-up, such as maintained blood pressure and improved healthcare delivery outcomes as nurses continue to educate patients in self-management of hypertension. 

Additionally, nurses can enhance knowledge about follow-up care among patients to promote timely care through subsequent check-ups and have the ability to convince patients to implement this change and reinforce it by consistent education and follow-up interventions appropriate for hypertensive patients (McGhee, 2023).

Conclusion

The growing statistics of hypertension among older people requires the use of healthcare information technologies to facilitate patient health outcomes. EHRs can be an effective informatics tool to promote patient care follow-ups and chronic care for hypertension management. The literature has also supported the use of EHR to analyze the data trends and patterns in evaluating hypertensive patients who practice follow-up care This is possible by implementing a change strategy by using the ADKAR model. Nurses can use this tool and change strategy to promote enhanced healthcare outcomes and improve patient safety among older hypertensive patients.
